Where can I find a stand alone disclaimer form for a primary beneficiary to disclaim in a BDIT/678 trust?

I have a client whose wife does not have enough assets to fund her exemption amount. He wants to give his wife a testamentary GPOA over his assets in his revocable trust to fund a credit shelter trust to the extent she predeceases him, how do I draft this?
